Contrast
Iodinated contrast
- GFR>30
- Okay if on dialysis
- Okay if ordering doc documents that benefit of CECT outweighs risk to kidneys
- Do not give in setting of suspected thyroid malignancy
Gadolinium-based contrast
- Gadavist GFR >15
- Eovist GFR >30, bilirubin <3
- Does not matter if they are on dialysis
- No gadolinium in pregnancy
Allergies
- If patient has a contrast allergy of any severity, they should undergo premedication regimen
- Exceptions:
- Patient is intubated
- Exam is emergent and ordering doctor accompanies patient during scan (in case of code)
Premedication regimens:
- IV: 200mg hydrocortisone 13, 7, and 1 hr prior, benadryl 50 mg 30 min prior
- PO: 50 mg prednisone 13, 7, and 1 hr prior, benadryl 50 mg 30 min prior
- Rapid: 200mg hydrocortisone q4h, at least 2 doses
Reactions
- Anaphylaxis: IM - 0.3 mL of 1 mg/mL epi (1:1,000), IV - 1 mL of 1mg/10mL epi (1:10,000)
- Vasovagal: 1L NS, atropine 0.6 mg IV if unresponsive
- Bronchospasm: 2 puffs albuterol
- Hives: 50mg benadryl
Extravasations
- As long as its <100cc, no need to consult plastics.
Fluoro requests

Only need to do emergent studies:
- Concern for esophageal rupture and unstable
- Post repair looking for leak
- Ileocolonic intussusception
- Neonatal bilious emesis

Remember always use omnipaque if concern for rupture/leak

All other studies can wait until Monday

Small bowel follow throughs can be done on the floor
- 200-300cc omnipaque

G-tube check can be done on the floor
- 30cc omnipaque (10 cc neonatal)

Retrograde urethrogram (rare) if pelvic fx and blood at meatus

- 20-30cc contrast, just until bladder visualized, 10F catheter
